Answer: The guidelines have recently changed. If you make content changes to books that have already been approved, they'll probably need some formatting changes made to them, as well before you re-submit.
The most common problem: not having a properly formed NCX file for the .epub format. This is done by coding the chapter headings with style tags, so that the iPad and other devices that utilize the .epub format can auto-generate a table of contents.
The auto-generated table of contents is not the same as the "visible" hyperlinked TOC that we've been including in most non-fiction books for the past year. The auto-generated TOC is not visible on the manuscript itself and accessed only through the e-reader software.
